Shut-in Home Delivery Service

[/vc_column_text][/vc_column][/vc_row][vc_row][vc_column][vc_column_text]Grateful Hearts’ Shut-in Home Delivery service is a service provided through our Food Distribution Program.  This service exists for individuals who do not have alternative options of obtaining food.

If you can afford to purchase food, we ask that you request delivery services from your local grocery store.

If someone in your household (or a friend or relative) can pick-up food from our pantry on your behalf, please call 562.431.0880 EXT 301 to make an appointment.

Unless you are qualifying because you are over 80, you will be required to provide a doctor’s note explaining why you need this service (Please see the note further below).

To qualify for this service, you must validate that:

1. Everyone in your household either: Has difficulty accessing food pantries because of disability, impairment OR extreme vulnerability to covid-19 due to age (80+), OR underlying condition

2. You do not have a nearby friend or relation that can go to a food pantry/grocery store on your behalf.

 

For individuals qualifying for a reason other than age, you will be required to submit a referral letter from a health care provider (doctor, nurse, physician assistant, etc.) ON THAT PROVIDER’S STATIONERY.

The letter should state that, “Due to physical or other impairments, ( client name ) is unable to use an existing Food Pantry.” An appropriate explanation of impairment(s) must be included as well as the name of the contact person and the phone number of this provider. Registration

Please complete the following registration process:

1. Complete New Client Application (click here).

2. Submit referral letter from a health care provider to Amy Ovando at aovando@gratefulhearts.org[/vc_column_text][vc_separator color=”pink”][vc_column_text] 

Once we have received all the required information, and you have been approved for the shut-in delivery service, a call center representative will contact you to schedule your food delivery date.
